The following link allows you to pay your Administrative Penalty Payments electronically. We accept credit cards and check payments. Click Here
You will need to fill in the following fields for billing information (information associated with credit card or bank account):

Zip
Then enter the information about the penalty. There’s a check box that allows the respondent name and address to update with the billing information entered above, if they are the same. Then enter:
- Agency Name (Company Name/Addressee per the order)
- License Number (NAIC No., Certificate No., if not applicable enter NA)
- Unit Name (The MIA Division issuing the penalty per the order)
- Case Number/File Number (included in the order)
- Contact Email Address (Who to contact with any payment related questions)
- Amount to pay (per the order)
- Clicking proceed will allow you to make the payment.
